How to Choose Your Tier

• Green Bottle (Full Price): You have stable income, own property, have savings, and can comfortably meet your needs without financial strain with some additional discretionary spending.

• Yellow Bottle (Middle Tier): You have a moderate income, may have some savings, but face occasional financial stress or limited discretionary spending.

• Red Bottle (Supported Tier): You experience financial hardship, have low or no income, rely on public assistance, or struggle to meet basic needs.

Green Bottle

For those with stable finances who can pay the full market rate.

Yellow Bottle

For those with moderate income facing some financial constraints.

Red Bottle

For those experiencing economic hardship or significant financial barriers.

Additional Notes

• Honesty-Based System: Please self-assess your tier based on your overall financial situation, including income, wealth, and expenses. No proof of income is required.

• Session Availability: A limited number of Red Bottle slots are available each month to ensure sustainability. Please book early.

• Custom Arrangements: If none of these tiers work for you, contact us to discuss alternative options, such as bartering or payment plans.

• Confidentiality: Your tier selection is private and will not affect the quality or duration of your session.

• All price examples listed in the above tiers pertain to one hour sessions. The price of additional durations are provided when booked online.

Late Policy

• Clients are encouraged to arrive 5 minutes prior to their scheduled appointment time to prepare for the session.

• If a client arrives late, the session may be shortened to end at the scheduled time to accommodate subsequent appointments.

• Clients arriving more than 15 minutes late may be asked to reschedule, and a cancellation fee may apply (see No Show/Cancellation Policy below).

Massage Termination Due to Misconduct

• Our therapists maintain a professional environment and reserve the right to terminate a session immediately if a client engages in inappropriate behavior, including but not limited to:

  • Sexual advances, comments, or gestures.

  • Verbal or physical harassment.

  • Any behavior deemed disrespectful or threatening to the therapist or staff.

• In such cases, the client will be required to pay the full session fee, and future appointments may be refused at the discretion of the establishment.

No Show/Cancellation Policy

• Appointments cancelled with less than 24 hours’ notice or failure to show up for a scheduled appointment will result in a charge of 50% of the session fee. We understand that emergencies and illnesses can arise, therefore last minute cancellations due to emergency, illness, or inclement weather will generally not result in any missed appointment charges, however this will be determined at the discretion of the therapist.

• Clients who repeatedly cancel with short notice or fail to show up may be required to prepay for future appointments or may be refused service.

• To cancel or reschedule, please contact us at least 24 hours in advance via phone or email if you are unable to modify your appointment online.

I voluntarily request and consent to receiving massage therapy.I understand that the massage service offered is for the purpose of general wellness, stress reduction, and relief of muscular tension only.

I do not have any injuries or conditions that prevent me from receiving massage therapy. I understand the importance of informing my massage therapist of all medical conditions and medications that I am taking, and that there may be additional risks based on my physical condition.

If I experience any pain or discomfort, I will immediately inform my therapist so that the pressure or techniques used can be adjusted to my comfort level. I will not hold my massage therapist responsible for any pain or discomfort I experience during or after the session.

I understand the risks associated with massage therapy include, but are not limited to:

• Superficial bruising

• Short-term muscle soreness

• Exacerbation of undiscovered injury

I do not have any contagious conditions that may put my massage therapist or other clients at risk.

I understand that I or the massage therapist may terminate the session at any time.

I have been given the opportunity to ask questions about massage therapy and my questions have been answered.

Information regarding massage in general, benefits, contraindications of massage, and possible alternative therapies have been explained to me. I further understand that massage therapy is not a substitute for a medical examination or treatment, and that I should see a physician or other qualified health specialist for any mental or physical ailment of which I am aware. I understand that massage therapists do not diagnose illness

or disease, and nothing said during the massage should be construed as such. My consent is informed and voluntary and I understand that I may withdraw my consent at any time except for actions already taken.

Voice Recording Acknowledgement and Consent

I understand that my verbal intake at the beginning of each appointment may be voice recorded for the purpose of accurate SOAP note charting. I understand that these recordings are for the sole purpose of accurate charting and will 

never be shared or distributed with the exception of court orders.
